Promoting Independence Co-ordinator

Age UK North Tyneside are looking to recruit a Promoting Independence Co-ordinator to work collaboratively within multi-disciplinary teams, both internal and external, to ensure the voice of the customer is heard, and coordinating a range of internal and external services and a team of volunteers to support older people to achieve self-set goals related to their wellbeing. This role is based in North Shields. 

The co-ordinators will meet with older people on a one-to-one basis and carry out a “guided conversation” which is used to draw out the needs and wishes of the customer. The co-ordinator then brings together all of the support and services necessary for the customer to achieve their goals, ensuring that the person is placed at the center of the service provision and coordinating this accordingly.

Requirements

Essentials:

  1. Good level of written English and Mathematics (GCSE grades A-C/4-9 or equivalent)
  2. Good working Knowledge of Microsoft Office, Email and CRM systems 
  3. Experience in the Voluntary Sector, NHS, Public Health or Social Care with a proven track record of using professional knowledge to bring about change
  4. Experience of developing and managing innovative projects to successful outcomes within time and budget parameter
  5. Full UK Driving License and access to car for work
  6. Ability to manage multiple activities and prioritise accordingly
  7. Clear and concise communication skills, both verbal and in writing

Desirable:

  1. Experience of older peoples’ issues
  2. Experience in the application of quality issues and good practice in relation to older people, the hospital environment and discharge process
  3. Experience of monitoring Health and Safety, monitoring & evaluating projects
