North Tandridge PCN
Our Primary Care Network (PCN) comprises of 5 practices, covering the North Tandridge area. The practices have a combined population of around 36,000 patients. There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. You will be joining an enthusiastic team of clinicians and administrators at Elizabeth House Medical Practice, and this role will contribute to improving the quality of care of our patients.
Within the North Tandridge PCN, we have a well-established Anticipatory Care Hub, a Children and Young Persons Social Prescribing/Health and Wellbeing Service and have good links with our local hospitals such as East Surrey Hospital and Caterham Dene. North Tandridge PCN continue to recognise the value that additional roles bring to the practices and patients and would like to build upon the existing team by employing a Prescribing Advanced Nurse Practitioner to work at Elizabeth House. Our aim is to provide exemplary patient care; find innovative solutions in general practice to deliver the best care we can for our patients.
Elizabeth House Medical Practice
Elizabeth House Medical Practice is based in Warlingham and has a registered list size of 6,100 patients. You will be joining a multi-disciplinary team of GPs, ANPs, Practice Nurses, Clinical Pharmacists, Pharmacy Technicians, First Contact Physiotherapists and Healthcare Assistants.
Job summary
Whilst General Practice has some challenging times ahead, this role provides an exciting and innovative opportunity for someone who wants to make a real difference in the communities they serve. You will work autonomously supporting patients within Elizabeth House Medical Practice under the Supervision of the GP Leads providing enhanced clinical assessment and treatment skills to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems and will play a vital role in increasing capacity for all appointments including home visits.
The successful candidate will be provided with excellent support, the opportunity for development in the role, and will be clinically supervised by a General Practitioner.
As part of their clinical duties, the successful candidates will join the multi-disciplinary team in providing comprehensive care and will work with patients, carers, and families.
The post holder is a Prescribing ANP who acts within their professional boundaries, supporting and working alongside general practice.
The post holder will work as an autonomous practitioner working both independently and as part of a multi-disciplinary team in a patient-facing role.
Primary duties and areas of responsibility
- To independently undertake clinical assessments, and use this in combination with the patient’s medical record, to diagnose, treat and discharge patients with a wide range of minor illnesses
- To ensure effective coordination and liaison with practice teams and, where appropriate, multidisciplinary teams across primary and secondary care services
- To adapt styles of communication appropriately to promote empowerment, self-management and health education for patients
- To attend clinical and team meetings as required
- To be a source of clinical expertise to other team members, both clinicians and those within an administrative function
- To provide specialist advice relating to minor illness to patients, relatives, carers and, where appropriate patients’ practices
- To identify actual or potential clinical risks and take appropriate action using, when required, the appropriate reporting structure
- To work within professional guidelines and codes of conduct
- Work within the guidelines set out in all relevant policies and protocols including infection control and safeguarding
Service Delivery Responsibilities - To ensure that patient records and documentation are maintained in line with regulatory standards, ABC policies and the use of the relevant clinical systems including documenting full and accurate records of assessment, treatment and aftercare
- Work within local protocols and guidelines
- To collect data and provide service information to the PCN board as required
- To contribute to the development and establishment of systems
- To ensure that Clinical Lead GP is kept informed of any service issues and potential risks
