Overview:

Saxonbrook Medical Centre are looking for an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met.

Whilst General Practice has some challenging times ahead, this role provides an exciting and innovative opportunity for someone who wants to make a real difference in the communities they serve. Overview:

Healthy Horley PCN has an exciting opportunity to expand their Primary Care Workforce by employing a CYPSP (Children and Young People Social Prescriber) and supporting the delivery of a CYP wellbeing service for our patients. We would like to build on our existing PCN team as we continue to recognise the value the additional roles bring to our practices and patients. Our aim is to provide exemplary patient care; find innovative solutions in general practice to deliver the best care we can for our patients

Within Healthy Horley PCN, there is an identified need for a Social Prescribing Link Worker, to work with Children and Young People, to support their health and wellbeing and helping them to navigate through the different support networks available to them. This includes linking them in with Sexual Health Services, Drug and Alcohol support and Mental Health support as required.

Our Primary Care Network (PCN) comprises of 3 practices, covering Horley and surrounding areas (Outwood, Salfords and Smallfield). The Practices have a combined population of 31,500 patients. There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. You will be joining an enthusiastic team of clinicians and administrators. The role will contribute to improving the quality of care of our patients across the network of practices.

Overview:

Saxonbrook Medical Centre are looking for a Pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a Senior Clinical Pharmacist and a GP who will develop, manage and mentor them.

The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice.

The post holder will provide clinical leadership on medicines optimisation and quality improvement, manage some aspects of the quality and outcomes framework and enhanced services and contribute to the delivery of the local medicine’s optimisation strategy. 

The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Overview:

Saxonbrook Medical Surgery are looking for a Paramedic to join their clinical team to focus on supporting patients in the community and housebound patients.

You will work closely with clinicians from the practice and will play a vital role in increasing capacity for all appointments.

Under supervision of the GP in the practice, the post holder will provide care for the presenting patient from focused history taking, clinical assessment, diagnosis, treatment, and evaluation of care. The post holder will demonstrate critical thinking in the decision-making process. The post holder will provide a holistic and clinical service, with support and supervision of GPs, implementing agreed management plans and following approved protocols as appropriate.

This post will provide first point of contact care for a range of patients presenting with undifferentiated, undiagnosed problems.

Overview:

The post holder will autonomously undertake assessment and treatment of same day urgent care patients who may present with complex and/or chronic illnesses and/or injury. This will be a portfolio role working across various services, including but not limited to ABC Remote Services (e-Hub and Enhanced Access), and Acute Primary Care Hubs.

The post holder will determine a clinical diagnosis, treatment and appropriate discharge as indicated as well as maintaining patient records, as well as be a source of clinical expertise to less experienced staff, enhancing skills and knowledge, and work in partnership within a multi-disciplinary team.

This is an exciting opportunity for the successful candidate to join the Haywards Heath Villages Primary Care Network (PCN) team in supporting a wide range of patients of all ages.This post is part of the Additional Roles Reimbursement Scheme for Primary Care Networks funded by NHS England and includes funded access to the Clinical Pharmacist Training Pathway for any further modules required to complete the pathway.

Overview:

The role is to provide clinical pharmacist support to all services including Enhanced Access clinics. The services are GP lead and are supported by a team of Clinical Pharmacists who must be Independent Prescribers. This will be a predominantly remote role. Standardised processes and protocols are in place.

Reporting to the Remote Service Manager and the GP Clinical Lead within the remote GP team for clinical supervision. This role will be part of a team consisting of GPs, ANPs, skilled administrators and the potential for other AHPs, working both at physical locations and remotely.

Alliance for Better Care Ltd will provide consistent employment support throughout the appointment.

As part of the clinical duties in both roles, the successful candidates will join the multi-disciplinary staff in providing comprehensive care and will work with patients, carers, and families. 

The post holder is a Pharmacist, who acts within their professional boundaries, supporting and working alongside general practice. In this role they will be supported by a GP who will develop, manage and mentor them.

The post holder will ensure that the service integrates with general practice and, where appropriate, secondary care, to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and supporting patients to manage their own health within their communities so requires motivation and passion to deliver excellent service within general practice. 

The post holder must be an Independent Prescriber.
