Job Description:
CRT Care Services is seeking dedicated and compassionate Care Workers to join our Home From Hospital Service team. This service supports individuals in their transition from hosppital to home, promoting independence through reablement and recovery ethos for all people accessing the service. You will be required to travel brough wide for this role.
Responsibilities:
- Provide support to individuals returning home from hospita
- Promote independence and recovery though personalised care plans
- Assist with daily living activities and rehabilitation excercises
- Monitor and report on the progress of individuals
- Collaborate with healthcare professionals to ensure comprehensive care.
Requirements:
- Previous experience in a care worker role is essential
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassion and patient demeanour
- Commitment to promoting independence and recovery
Pay and Shift Patterns:
Care Workers will be paid in block hours and shift patterns: The hourly rates are as follows;
- Monday to Friday : £13.60 per hour
- Saturday to Sunday: £14.20 per hour
Location:
Rotherham - Borough wide
- Fleet cars available if required

Qualifications required
- Valid driving licence and access to own vehicle
- Ability to follow care clans effectively
- Strong communication skills
