Reablement Risk Assessor - West Sussex 

Salary: £18,360.00 £ 25,489.80

As a Reablement Risk Assessor you will carry out customers’ first visit, this can include assisting/reenabling with personal care, breakfast & medication. You will then complete the risk assessments which will be person centred and focus on the needs of the customer to include Personal Care, Medication, Mobility & Environment

A full induction course and ‘shadow’ training will be provided to the successful candidate.

You will be required to work 37 hours per week. Shifts will be 08:00 – 16:00 predominately Mon – Fri however flexibility is required to include weekends on a rolling rota.We are looking for exceptional people who share our values and our commitment to helping and caring for individuals in our community.
