Hours: 37 hours per week to include weekend rolling rota

The role

Do you have a passion for helping others and making a difference to someone’s life?

As a Trusted Assessor you will carry out or be present at the first visit with the customer within their own home.  Your initial assessment/Goal setting will form part of the care plan which will enable the Community Care Assistant to provide the right level of support to aid the customer to become as independent as possible.

Key Responsibilities

  • Completing the Assessment/ Risk Assessment - which will be person centred and focus on the needs of the customer which could include Personal Care, Medication, food preparation and Mobility
  • Assess the customer home & environment
  • Ensure the promotion of independence, dignity and quality of life
  • You will need to gain and maintain a comprehensive knowledge of available local community resources and proactively liaise with Occupational Therapists, Social Workers and Health Professionals, to maximise the support the sport available to the individual.

Key Requirements

  • Driving licence and use of personal vehicle for work purposes
  • Enhanced DBS Check (can be arranged through ECL if needed)
  • Complete a Care Certificate qualification through ECL during your probationary period.
