Outreach Support Worker

Fair Ways is a leading children’s services charity based in the Solent, delivering Fostering, Education, Residential, Mental Health and Family services and cares for over 400 children, families, young people and vulnerable adults each year.

Where and when do we need you? 

Outreach Support offers a wide range of services varying from daily support within the family home to weekly visits for activities within the community. The service is bespoke to each Service User, with consistency of staffing a major factor in planning for all support. Dealing with complex health needs, physical disabilities, learning disabilities, Autistic Spectrum Condition and behavioural needs.   

As a Support Worker you will be involved in exciting and rewarding things like: 

  • Providing flexible support in the family home and/or community to service users with disabilities, complex health needs and behaviour that challenges. 
  • Actively involving Service Users in decisions about their care plan in line with their level of understanding, in order to maximise their choices. 
  • Providing one to one support (unless two staff are required based on service user risk needs) and be proactive in planning and carrying out activities and skills development in the community setting. 
  • Contributing, adhering to and regularly updating person centred care plans, risk assessments and management plans of the Service Users, in conjunction with colleagues and other professionals, to promote and safeguard the welfare of the Service Users. 
  • Be happy undertaking small elements of personal care in line with the needs of the individual although this is by no means the primary function of our outreach support workers. 

This role is offered on a part-time or full-time permanent basis. All visits are a minimum of one hour and can range up to whole days. Support Workers will never work less than a 3 hour duration as part of their shift pattern. Travel time and mileage is paid in addition! 

Desirable…. but not Essential 

Whilst we value the knowledge and understanding that experience and qualifications can bring we also know that some of our best people have come to us from unrelated backgrounds. These people already held our core values at heart, and with the dedication of our amazing in house training and development team, we have supported the rest! 

  • Previous experience of working with young people and/or adults with health/social care/educational needs.
  • A Level 3 qualification in childcare/social care.
