To assist those we support to lead full, active and socially connected lives and support people to reach their personal outcomes.
Our teams take responsibility for making their own decisions about how they organise their work and how they decide on how they’re going to get their work done. This role therefore requires good interpersonal skills and an ability to set and meet targets, ability to ask for and receive accurate feedback and ensure that cover is managed by the team at all times (e.g. holidays, sickness, training).

Develop an understanding of person centred working and work from its value base at all times. |

Contribute to ongoing use of ‘My Life, My support plan’ by keeping details updated and relevant. Contribute to the yearly review of ‘My Life, My Support Plan’. |

We are recruiting a new team of Wellbeing Practitioners for someone that we have not started working with yet. We will need to recruit a balanced team to work with someone to support them to transition in to their own home as they have been in shared accommodation.
This is a female* only service. Sleepovers will be included in the shift patterns.
This lady is very independent and the team should support her to get back to not relying on support. She will initially have support all of the time but we will have to build a relationship with this lady and understand when she would like time on her own and we would need to be creative with how we do this.
This lady is a very creative person. She enjoys art, music, singing, people full of life, humour, food, and lattes.
This lady comes across as someone that does not require much support. She will need support with everyday life skills, remembering events and appointments and keeping healthy.
