As we continue to grow, we are looking for a motivated, organised, and compassionate Care Coordinator to join our friendly office team.


💼 About the Role

As a Care Coordinator, you’ll play a crucial role in ensuring our complex care packages run smoothly and efficiently. You’ll work closely with clients, families, healthcare assistants, and clinical teams to ensure that every client receives consistent, high‑quality, person‑centred support.

Key Responsibilities

  • Scheduling and coordinating care teams to meet client needs
  • Building strong, supportive relationships with clients, families, and staff
  • Ensuring care plans are followed and compliance standards are met
  • Supporting recruitment, onboarding, and ongoing staff development
  • Maintaining accurate records and documentation
  • Participating in a shared weekend on‑call rota
  • Responding to urgent staffing or client needs calmly and professionally

What We’re Looking For

  • Previous experience in care coordination or a similar administrative role
  • Excellent communication, organisation, and time‑management skills
  • Strong IT literacy and confidence using scheduling systems
  • Ability to stay calm and solution‑focused in fast‑paced situations
  • A genuine passion for delivering exceptional care
  • A proactive, positive attitude and willingness to support the wider team
