Saxonbrook Medical is a 20,000 patient population practice serving our community over two sites. The team is comprised of Doctors, Physician Associates, Paramedics, Pharmacists, Nurses and Healthcare Assistants all supported by a strong administrative team.  We work together as a team to provide the best care possible to our community. We are a teaching Practice who welcome a number of trainees into the Practice including Doctors, Student Physician Associates and Student Nurses and also support the progress and development of our employees.

We work within a local Primary Care Network, working collaboratively with Social Prescribers, Pharmacists and First Contact Practitioners.

We offer the NHS Pension Scheme. Clinical staff receive 25 days annual leave + bank holidays pro rata. There is also a provision for study leave.

Job summary

The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met.

Whilst General Practice has some challenging times ahead, this role provides an exciting and innovative opportunity for someone who wants to make a real difference in the communities they serve. The successful candidate will be provided with excellent support, the opportunity for development in the role, and will be clinically supervised by a General Practitioner.

Primary duties and areas of responsibility

  • To independently undertake clinical assessments, and use this in combination with the patient’s medical record, to diagnose, treat and discharge patients with a wide range of minor illnesses
  • To ensure effective coordination and liaison with practice teams and, where appropriate, multidisciplinary teams across primary and secondary care services
  • To adapt styles of communication appropriately to promote empowerment, self-management and health education for patients
  • To attend clinical and team meetings as required
  • To be a source of clinical expertise to other team members, both clinicians and those within an administrative function
  • To provide specialist advice relating to minor illness to patients, relatives, carers and, where appropriate patients’ practices
  • To identify actual or potential clinical risks and take appropriate action using, when required, the appropriate reporting structure
  • To work within professional guidelines and codes of conduct
  • Work within the guidelines set out in all relevant policies and protocols including infection control and safeguarding

Service Delivery Responsibilities

  • To ensure that patient records and documentation are maintained in line with regulatory standards, ABC policies and the use of the relevant clinical systems including documenting full and accurate records of assessment, treatment and aftercare
  • Work within local protocols and guidelines
  • To collect data and provide service information to the PCN board as required
  • To contribute to the development and establishment of systems
  • To ensure that Clinical Lead GP is kept informed of any service issues and potential risks
