Horsham Central PCN

Horsham Central Primary Care Network is a NHS Collaboration between two GP Practices – Holbrook Surgery and Riverside Medical Practice - working together to provide enhanced access services.

Our surgery teams work closely, sharing expertise and resources to develop new services. Our vision is to continue to improve the quality of care that we provide in alignment with the needs of our patient population.

Our Primary Care Network builds on the existing primary care services and enables a greater provision of proactive, personalised and more integrated health and social care. We are supported by practitioners in additional roles who allow us to create bespoke multi-disciplinary teams based on the needs of our local population. By working together with local community services, this allows us to make support available to people where it is most needed.

Supervision

Regular supervisions on a one-to-one basis including an annual formal appraisal are held with a line manager. If you are in a patient-facing role, you will also receive regular Clinical Supervisions with a Clinical Supervisor.

Job summary

The objective of the Business and Transformation Manager is to provide a central source of leadership and management to aid service transformation for our population.

The ideal candidate will need to be proactive, have a passion for Primary Care, and have a proven track record in delivering Primary Care improvements.

The remit of this role is broad and will require attention to detail in delivering high-quality day-to-day operations.

You would support increased access to care for patients on a range of integrated digital transformation projects that would enable the staff to work more effectively and help to sustain the future of general practice services.

As Business and Transformation Manager you would work within the PCN to lead and develop a strategic plan to build a robust infrastructure for Horsham Central PCN, which: 

  • Is underpinned by a quality improvement methodology
  • Drives integrated working

  • Reduces variation between practices
  • Is underpinned with easy-to-understand data packs to support decision-making that includes but is not limited to demand, capacity, utilisation data, staff, and patient experience

Primary duties and areas of responsibility

To work closely with the Practice Managers in developing the PCN’s use of new services, processes and technology to improve overall efficiency in a range of projects including but not limited to, cloud telephony, digital triage, promoting the use of the NHS App, and optimising the use of clinical systems.

Strategic Planning

  •  To lead and develop a strategic plan that includes a robust digital infrastructure
  • Support PCNs in holding their own mission and plan for delivery, translating this to a business plan against which they can track progress
  • To clarify objectives and priorities for network delivery in line with operational capacity, financial parameters and contract requirements
  • Promote the engagement of all PCN practices and act as an ambassador for the network
  • To proactively build relationships with system leaders to help influence proactive and new models of care
  • To use business intelligence tools to aid data-driven decisions

Business Operations

  • To provide oversight and accountability to the delivery of PCN contracts and specifications
  • Identify and record risks and issues and develop contingency plans with Network Leads
  • Ensure all relevant national strategies, policies, targets, and frameworks are fully implemented
  • To drive issues through to resolution, acting as a point of expertise
  • To support the collation of financial information, compiling and presenting financial reports 
  • To ensure that the PCN's physical and financial resources are managed and maintained for maximum effectiveness
  • Manage allocated staff, including recruitment, induction, setting workload priorities, delegating tasks, day-to-day management, including appraisal/ investigations etc
  • Work to establish teams of ARRS staff within PCNs, supporting PCNs to identify workforce need and develop or recruit staff in to this
  • Ensure ARRS staff have all necessary support to thrive in their role including objective setting with the PCN lead and providing a regular touch point for any concerns
  • Support or establish where not already in existence networks of ARRS roles of specific disciplines between PCNs
  • To foster a culture of collaboration and promote learning to drive continuous improvement
  • To take an active role in the development and embedding of the PCN's culture, values and reputation
  • To represent the network at external meetings
  • To facilitate progression for the PCN staff to promote retention

Project management

  • Drive improvements in patient access, service, and experience
  • Ensure that digital and operational transformation is embedded and aligned with system and national strategies in collaboration with the ICB Digital Transformation team
  • Manage the set up and delivery of multiple projects working with colleagues and experts to scope, plan, design and oversee effective delivery
  • Provide expertise of best practice change and project management methodologies

Analysis

  • Interrogate and analyse data to assess demand, capacity, and activity to support the delivery of clear project reporting and project outcomes
  • Submit regular high-level project reports showing progress against milestones, status, resource requirements, risks and dependencies and recommend a range of options
  • Develop key performance indicators and milestones for specific programmes and projects, managing conflicts and ensuring interdependencies are managed effectively
  • Create and maintain accurate documentation in relation to finances, standard operating procedures, contracts, contacts, and decision-making, which is accessible centrally

Service improvements

  • Hit government set targets and support population health management
  • Identify opportunities for streamlining the service within the PCN
  • Enable PCN staff to maximise clinical efficiency and enhance internal and external communications
  • Embed patient and public involvement at all levels of decision making

 Communication

  • Engage and develop effective communication channels with key strategic policy makers to influence policy making
  • Communicate the vision for the PCN and support the development of strategy and operational policies to support this vision
  • Act as a champion for patients and their interests and involve the public in the policy development and decision-making
  • Build relationships and facilitate cooperation between the practices and at neighbourhood level to improve patient care delivery
