Overview:

The objective of the Business and Transformation Manager is to provide a central source of leadership and management to aid service transformation for our population.

The ideal candidate will need to be proactive, have a passion for Primary Care, and have a proven track record in delivering Primary Care improvements.

The remit of this role is broad and will require attention to detail in delivering high-quality day-to-day operations.

You would support increased access to care for patients on a range of integrated digital transformation projects that would enable the staff to work more effectively and help to sustain the future of general practice services.

As Business and Transformation Manager you would work within the PCN to lead and develop a strategic plan to build a robust infrastructure for Horsham Central PCN, which:

Overview:

This role is to support the smooth co-ordination of patient care for one practice within the Primary Care Network for the benefit of our patients.

The Care Coordinator will be responsible for consulting with patients and determining their needs, developing care plans, coordinating patient-care services, educating them about their condition, empowering them to be independent whenever possible and working with the care team to evaluate interventions.

Overview:

Alliance for Better Care CIC is a GP Federation that unites 47 NHS GP practices across 12 Primary Care Networks in Sussex and Surrey. We support our Primary Care colleagues as well as their patients, to transform how healthcare is managed within the community.

As part of our friendly Equity team, your focus will be on delivering essential healthcare services to the underserved parts of our community and to people who face additional barriers to accessing healthcare.  We are proud of our Equity work to date and this is a fantastic opportunity to really make a difference to people's lives.

You will be responsible for delivering a growing range of services such as health assessments, immunization, health checks, opportunistic wound dressings, health promotion and prevention of ill health. The aim is to reduce inequity for vulnerable cohorts and communities who haven't previously engaged with primary care.

You will be required to visit people at home, specific sites and community-based settings across East Surrey and West Sussex. Cohorts included are Rough Sleepers, Homeless, Asylum Seekers and Refugees, Gipsy, Roma and Travelers and other underrepresented Ethnic minorities, 

The role is offered on a fixed-term contract until end January 2025, on a full-time or part-time basis of 3 to 5 working days per week.

Overview:

The post holder will work with an established Clinical Pharmacist team across the PCN and will be assigned a lead clinical and operational contact. This post is part of the Additional Roles Reimbursement Scheme for PCNs funded by NHS England and includes funded access to the Primary Care Pharmacy Education Pathway pilot for pharmacy technicians.

The ambition is to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice.

The post holder is a Pharmacy Technician, who acts within their professional boundaries, supporting pharmacists in general practice to ensure effective and efficient use of medicines. In this role they will be supervised by a pharmacist who will develop, manage and mentor them.

The Pharmacy technicians core role responsibilities will cover clinical, technical and administrative categories.

The post holder will work as part of a multi-disciplinary team and this role involves provision of technical support for the pharmacy service to ensure professional operational standards and excellence in pharmaceutical care and safety are maintained.

The role will support the practices within Haywards Heath Central PCN to ensure the delivery of safe, effective and efficient systems for medicines related clinical correspondence; repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.

Overview:

This is an exciting opportunity for the successful candidate to support a wide range of patients of all ages. This post is part of the Additional Roles Reimbursement Scheme for Primary Care Networks funded by NHS England.

The successful candidate will be an experienced pharmacist, who acts within their professional boundaries and has completed the CPPE pathway. They will work as part of a multi-disciplinary team in a patient-facing role; take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy. In particular, older adults, people in residential care homes and those with multiple co-morbidities.

 

The successful candidate will provide support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescriptions system, deal with acute prescription requests, medicine reconciliation on transfer of care and systems for safer prescribing. They will provide expertise in clinical medicines advice whilst addressing both public and social care needs of patients registered with the practice.

 

The successful candidate will provide clinical leadership on medicines optimisation and quality improvement, manage some aspects of the quality and outcomes framework and enhanced services and contribute to the delivery of the local medicines optimisation strategy. They will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.

 

This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Overview:

We are looking for a Social Prescribing Link Worker to join our team. Park and Orchard PCN in Horsham is a new PCN with a population of 31,000 patients. We have a higher than average population of elderly patients, many of whom live in their own homes and require a lot of social support. There are challenges for the whole population in terms of the cost of living, social isolation and loneliness, housing, employment and mental health issues. We also have higher than average numbers of young people struggling with mental health issues and we have a good working relationship with the iRock cafe in Horsham.

During the last 5 years as part of Horsham Central PCN we have had a strong focus on personalised care, building our team of social prescribers, health coaches and care coordinators alongside our mental health support workers. We led on developing our Horsham District Befriends community service and obtained funding in order to address social isolation after the Covid pandemic. We are also working in partnership with Horsham Wellbeing hub and other local community organisations to offer support to those struggling with the peri/menopause. We run a partnership Menopause Cafe and Menopause Information Sessions. We also have a strong focus on dementia, young peoples mental health and cost of living and are keen to develop these further.

Our new PCN will be developing a multidisciplinary Frailty Team which will include our social prescriber.
