Overview:

Saxonbrook Medical Centre are looking for an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met.

Whilst General Practice has some challenging times ahead, this role provides an exciting and innovative opportunity for someone who wants to make a real difference in the communities they serve. The successful candidate will be provided with excellent support, the opportunity for development in the role, and will be clinically supervised by a General Practitioner.

Overview:

This role is to support the smooth co-ordination of patient care across practice  for the benefit of our patients. The post holder will join a team of PCN Care Coordinators. The Care Coordinator will be responsible for consulting with patients and determining their needs, developing care plans, coordinating patient-care services, educating them about their condition, empowering them to be independent whenever possible and working with the care team to evaluate interventions.

This role will be expected to heavily support the practice to reach its targets for their Enhanced Services and Quality Outcome Framework (QOF).This role will need to support (where necessary) all additional projects led by NHSE or the ICB which will support the needs and care of the practices patient list.

Overview:

Saxonbrook Medical Centre are looking for a Pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a Senior Clinical Pharmacist and a GP who will develop, manage and mentor them.

The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice.

The post holder will provide clinical leadership on medicines optimisation and quality improvement, manage some aspects of the quality and outcomes framework and enhanced services and contribute to the delivery of the local medicine’s optimisation strategy. 

The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Overview:

About our Occupational Health Service

Our Occupational Health Service is newly launched and is designed to prioritise employees’ health and wellbeing from the moment they start work.

The service caters for both internal staff and external clients and includes the full remit of Occupational Health practice including:

  • pre-employment medical screening
  • management referrals, interviews, and examinations
  • vaccinations – including Tetanus / Polio / Diphtheria, Hepatitis B, and MMR

We are currently in a growth phase and our aim is to continue this growth by expanding our dedicated and experienced team in order to continue delivering an exceptional service for our staff and clients.

The post holder will:

  • Provide effective day to day professional advice across the service, leading, organising, and managing OH nurse clinics and workload, reporting to the Clinical Lead Nurse on general progress and around any areas of difficulty or need.
  • On a daily basis support the work of the Occupational Health team and liaise with the Service Manager regarding progress and around any areas of difficulty or need.
  • Provide the highest standard of clinical care, ensuring that all clinical standards are met and where necessary taking appropriate corrective action.
  • Provide a visible, accessible and knowledgeable presence in the service to whom service users and managers can turn for assistance, advice and support.
  • To participate in relevant training, health education and induction programmes as required.
  • To ensure Continuing Professional Development is achieved in line with NMC and revalidation guidelines

Overview:

Saxonbrook Medical Surgery are looking for a Paramedic to join their clinical team to focus on supporting patients in the community and housebound patients.

You will work closely with clinicians from the practice and will play a vital role in increasing capacity for all appointments.

As a Paramedic you will be providing a range of services such as assessment, diagnosis, treatment, telephone triage, clinical decision-making and referring patients appropriately under supervision. You will report to a GP/Partner who will provide support and mentorship.
