Overview:

Saxonbrook Medical Centre are looking for an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice teams to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required. In order to work at this level, NMC requirements for advanced practice must be met.

Whilst General Practice has some challenging times ahead, this role provides an exciting and innovative opportunity for someone who wants to make a real difference in the communities they serve. Overview:

Saxonbrook Medical Centre are looking for a Pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a Senior Clinical Pharmacist and a GP who will develop, manage and mentor them.

The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice.

The post holder will provide clinical leadership on medicines optimisation and quality improvement, manage some aspects of the quality and outcomes framework and enhanced services and contribute to the delivery of the local medicine’s optimisation strategy. 

The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

Overview:

Since early December 2021 ABC have led the way in mobilizing the COVID-19 vaccination programme and have delivered over 1,000,000 vaccinations to date.  Ensuring equity of access is a core part of ABC’s values and we have ensured vaccine delivery to those who may not access major sites or use online booking since the start of the program.  As we have progressed through the cohorts, we have identified multiple disadvantages to them. When delivering vaccines, we have since taken a more targeted approach in order to address health inequalities, provide NHS health care checks and Equity wellbeing checks. Therefore, we are looking to appoint an equity engagement advocate to support this essential work, build on the trust and attract new cohorts by providing excellent engagement techniques.

We are building on the learning from the vaccine programme to pilot a new mobile vehicle, which forms part of the existing Crawley Programme. This unit will go out to the streets of Crawley offering cardiology and respiratory testing, with a view to reach those in the most deprived areas of Crawley, and increase screening uptake, helping the Crawley population live well for longer. 

You will be driving the Mobile Van from ABC’s base in Horley to a weekly set location daily. Upon arrival you will be responsible for setting up the van, for example, putting the staircase up and unloading the van. The SASH team will meet you at the location and will be using the van to deliver their respiratory and cardiology service. Your role will be to engage with booked patients and also reach out to local vulnerable communities promoting this service and the equity services.  

The ideal candidate will understand the local area, have experience of working for the NHS in an engagement or advocacy capacity, strong communication skills and a positive and resourceful attitude with empathy being one of the most important core values.  You will be well supported with a strong team of Coordinators, Managers, Clinical Leads and good connections to community engagement and link worker teams and resources.  This should be a fulfilling and enjoyable role.   

The post is likely to involve some evening and weekend working to support outreach sessions, hours will need to be worked flexibly but overtime rates will be paid for anything over 37.5 hours per week.

Overview:

Saxonbrook Medical Surgery are looking for a Paramedic to join their clinical team to focus on supporting patients in the community and housebound patients.

You will work closely with clinicians from the practice and will play a vital role in increasing capacity for all appointments.

They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the service. The post holder will demonstrate critical thinking in the decision-making process. They will work collaboratively with the healthcare team to meet the needs of the patients, supporting the delivery of policy and procedures.

The post holder will provide a holistic and clinical service, with support and supervision of GPs, implementing agreed management plans and following approved protocols as appropriate.

This post will provide first point of contact care for a range of patients presenting with undifferentiated, undiagnosed problems.

Job purpose                

To support the PA Director, working alongside the Regional Clinical Director and Manager and in partnership with the NHSE SE Primary Care Schools providing effective clinical leadership, and contributing to the strategic development of the PA School.

Whilst there are primary care employers [GPs and Primary Care Networks] in the SE geography who proactively engage with the Physician Associate workforce and agenda, this is not replicated across the whole region, and there remains a core need for an infrastructure which can facilitate and deliver workforce transformation. The SESPA GP Advisor will therefore need first-hand experience and knowledge of Physician Associates in Primary Care, their skills and competencies, and how they contribute to the MDT.

The successful applicant will be expected to have a medical background and experience of leadership in a community or primary care setting and remain registered with the GMC with a license to practice as a GP in Kent, Surrey or Sussex.  They should be able to work remotely and from the office at East Surrey Hospital and able to travel across the region if required.

The successful applicant will be on Secondment to Surrey and Sussex Healthcare NHS Trust on a 12 month fixed term contract to cover Maternity Leave.

Background                

The South East School of Physician Associates was established and funded by HEE SE to enable and support delivery of excellent healthcare by Physician Associates across the South East, through the education, training, development, employment, deployment, and retention of the profession. The SE PA School covers the breadth of the region, operating out of two hub sites [SASH NHS Trust, and RBH NHS Trust]; core faculty for the SE PA School is identical in each hub site and postholders are regarded as members of a SE-wide team but with some specific geographical responsibilities. Postholders are expected therefore to work closely with their counterpart and the team in the other hub site. SESPA GP Advisors are responsible for the overall success, vision and leadership of the SE PA School. The vision, developed in conjunction with the Regional Clinical Directors and NHSE Senior Responsible Officer, should guide the rest of the team in what they should do and how they should do it.
