JOB PURPOSE: 

Are you a passionate and motivated individual, who is looking to further their knowledge in Health and Wellbeing? We have an exciting new role within our integrated wellbeing contract as a Smoking Cessation Practitioner.

The post holder will be responsible for delivering evidence based, structured and tailored support to all smokers across Cheshire West and Chester at multiple locations and settings. Previous experience in the role is not essential as full training and support can be provided.  

As part of the holistic care we provide to all our clients, the post holder will support all aspects of Health and Wellbeing delivery, including the facilitation of brief interventions on a wide range of health and wellbeing topics and signposting to additional support services both within and outside of the Integrated Wellbeing contract. 

The post holder will also be required to attend and support the delivery of outreach sessions and training as identified by the line manager.  Working closely with the wider team and other referring agencies, the post holder will ensure a smooth pathway for clients entering any of our health and wellbeing services. 

The post holder will be required to create and deliver training sessions across Cheshire West and Chester once they are established within the role. Therefore, the ideal candidate will be flexible in their approach, have a willingness to learn, a willingness to travel across Cheshire West and Chester, a confident public speaker and, ideally, have experience in behaviour change or coaching and/or have previously attended one of our Very Brief Advice sessions.

The post holder will be responsible for the dispensing and management of smoking cessation pharmacotherapy prescriptions and must ensure all service delivery is provided in line with contract specifications and medicines management. 

You must also have an interest in health improvement and a commitment to helping the wider business grow and thrive.

MAIN RESPONSIBILITIES (but not limited to)

Operational 

Practitioner responsibilities

Deliver face-to-face, group and telephone-based support between the hours of 9am-8pm Mon-Fri to clients entering the service as required (some weekend work may be required).

Ensure all data is accurate, kept up to date and support service audits. 

To engage with smokers contacting the service and provide evidence-based motivational support and evaluate their smoking status, willingness to quit and establish their quit plan appropriately.   

Provide compassionate interventions and signpost or refer people (as appropriate) to self-help resources, community support or specialist services.   

To build and maintain good working relationships with community staff and organisations and develop new pathways and processes.   

To follow-up clients referred/signposted in line with the service specification to assess progress, identify any problems and determine if further support is needed. 

To arrange and attend a range of meetings, conferences, workshops and outreach events as required, relating to smoking cessations representing Brio Leisure. 

Develop and maintain knowledge of national, regional and local health initiatives, promoting early intervention and preventative initiatives throughout the Borough. 

Training responsibilities 

Identify training needs within an organisation through consultation to understand their training requirements.

Deliver Smoking Cessation training programs (outsourced and/or in-house) and actively feed into their continued development.  

Devise new learning plans and resources, keeping up to date with latest trends and developments. 

Conduct evaluations and feedback to identify areas of improvement.

Market available training courses to organisations and provide necessary information about sessions. 

Finance and Administration 

Work within the allocated budgets and assist the service lead in providing information to ensure that targets and performance indicators are met.

To ensure all data is processed in a secure and confidential manner. 

Provide true, detailed and accurate accounts of interventions including providing accurate records of pharmaceuticals administered.  

Complete appropriate onward referrals to external organisations with the relevant information required.

Provide feedback to referrers relating to complex individuals who may need a multi-disciplinary approach.

Health and Safety 

Ensure all pharmacotherapy, outreach equipment and fittings associated with the role remains secure and appropriately temperature controlled at all times.

Ensure all relevant Health and Safety requirements are met when delivering projects in the community and that risk assessments are in place for all activity delivered. 

Other 

To carry out any other relevant duties as required, within the scope of the post. 

To actively engage with the wider developments of Brio with a keen interest of personal development.

Ability and willingness to travel across the Cheshire West patch to attend meetings, cover clinics and any other duties within the organisational need.

The nature of the work may involve the jobholder carrying out work outside of normal working hours from time to time.
