Care Coordinator (Part Time)

Working as an Out of Hours Care Coordinator, you will be based from our Gateshead (Felling) offices although due to the nature of the role then you will be expected to complete home care calls as and when required in the South Shields, Boldon and Fellgate areas.

Role is to work every "OTHER" Saturday and Sunday "ONLY" and the shift times are 6am to 2pm and 2pm to 1030pm on a rotational basis (paid at £14.50 per hour)

Optional extra days / hours are available during the week (as a Home Care Worker) which is optional (paid at £13.90 per hour)

A full UK driving license and access to your own vehicle is ESSENTIAL for this role

We are seeking a dedicated and organized Out of Hours (OOH) Care Coordinator to join our team to cover the South Tyneside area. The OOH Care Coordinator will be responsible for managing and coordinating care services to ensure that our clients receive continuous and reliable support.

Key Responsibilities:

  • Coordinate and manage care services during
  • Respond to emergency calls and provide immediate assistance as needed
  • Schedule and allocate care staff to meet client needs
  • Maintain accurate records of all care activities and communications
  • Liaise with clients, families, and healthcare professionals to ensure seamless care delivery
  • Provide support and guidance to care staff during out-of-hours shifts

Requirements:

  • Driving license and access to a vehicle is essential
  • Previous experience in a care coordination or similar role required
  • Excellent organizational and communication skills
  • Ability to work independently and make decisions under pressure
  • Compassionate and empathetic approach to client care
  • Proficiency in using care management software and other relevant tools
  • Availability to work weekends
