Overview:

Job Summary

The Community Link Worker will support people in a locality to make a positive difference to their personal and community assets.  The Link Worker will take a person-centred approach and support people identify issues affecting their health & wellbeing; identify personal outcomes and priorities; overcome barriers to achieving goals; support access & engagement with local and national agencies & services.

The Link Worker will also work to connect primary care, social care services, 3rd sector agencies and community resources to provide person-centred, joined-up solutions for people that promote health and wellbeing.

The Link Worker will manage team members as required, will provide regular support and supervision and will provide regular reports to external agencies on outcomes achieved.

Overview:

The Director of Services will join the Penumbra Leadership team, which reports to the Board of Penumbra, and which currently comprises our Chief Executive, Director of Corporate Services and Director of Programmes. Overview:

The In Care Survivors’ Alliance hosts two initiatives that contribute to the Scottish Human Rights Commission’s Action Plan on Justice for victims of Historic Abuse of Children in Care:

  • Future Pathways, a national support fund for survivors of abuse or neglect in care in Scotland, and;
  • Redress Support, which provides assistance to survivors who apply for financial redress.

Penumbra will employ the Link Workers who will work with the Alliance. Penumbra is committed to the values of  the Alliance and is a leading provider of person-centred, trauma-informed services that provide compassionate responses to people in distress.

Link Workers will provide the support to those who contact, or are referred, to the service. The aim is to provide short-term support, and to arrange supports, from a pre-defined menu of options where support requirements appear to require more than the initial support conversations.

Aberdeen WELL (Well Empowered Linked Locally) will provide direct access to a community-based team of Practitioners and volunteers.  The team will work with Partners to provide short-term support and will connect people to appropriate support agencies and community resources. The team will also develop peer networks so that people have access to support within their local areas from people with lived experience.

Overview:

This is an exciting opportunity to join the delivery of the DBI approach, which has widely been recognised as innovative and pioneering across Scotland.

DBI is a two-tiered response; a Level 1 response from frontline partnership agencies and Level 2 supportive interventions delivered by Penumbra. A Recovery Practitioner will respond to a referral made on behalf of a person experiencing distress within 24 hours, with the provision of time limited support. The Practitioner will then deliver community-based problem solving support, wellness and distress management planning, supported connections and signposting for a period of up to 14 days.

The role of Relief Recovery Practitioner is to assist people who use the service to establish and maintain a meaningful and fulfilling life in the community. Relief Recovery Practitioners play a central role in the planning and provision of quality, recovery focused support to people who use the service and promote self-management and opportunities for improved health and wellbeing.
