Our Constitution Street service delivers a high-quality Care at Home and Housing Support service for adults with severe and enduring mental health challenges. Supporting 11 individual tenancies, and community-based outreach support, the service operates on a 24/7 basis.
With the aim of equipping people with the skills needed to live independently and meaningfully within the local community, a strengths-based coaching approach to recovery is used to build confidence, maintain motivation and promote resilience. Alongside this we also provide practical support with food preparation, medication, budgeting and other daily living skills. Promoting participation and inclusion is a core value of the service, through working alongside people to deliver innovative, creative and personalised interventions and opportunities.
You will be experienced and passionate about working in a person-centred way with adults who experience mental health issues or adults who are expressing their needs through distressed behaviour and substance misuse. With a strong interest in people and a commitment to helping others, you will strive to make a positive impact on people’s lives and tailor support to enable people to fulfil their best potential.
As the service operates on a 24-hour basis, a variety of shifts will be required on a rota basis, including sleepovers which are paid at an additional rate.
