Hours: Full-time (37.5 hours per week, flexibility required including evenings, weekends and participation in the on-call rota)

Key Responsibilities

  • Promote a caring, supportive environment that meets the physical, emotional, social, intellectual, and spiritual needs of Service Users.
  • Ensure each Service User receives personalised care tailored to their individual needs.
  • Implement the requirements of the Health and Social Care Act 2008.
  • Manage Service User care packages effectively.
  • Supervise and support Care Workers to maintain high standards of care.
  • Liaise with professionals, Service User networks, and relevant community partners.
  • Coordinate care delivery, including:
    • Allocating appropriate travel time between visits
    • Arranging shadowing for new staff
    • Ensuring punctuality and correct care worker assignment
    • Monitoring ECM logs and compliance
    • Cross-referencing actual vs. planned bookings

Duties

  • Participate in practical care delivery as part of the rota.
  • Provide shadowing support to new or developing care workers.
  • Contribute to the carer buddy system to encourage peer support.
  • Attend Service User initial assessments, identify needs and risks, and determine suitability for care.
  • Negotiate fees with purchasing authorities or families when required.
  • Ensure Service Users receive essential information and documentation (e.g., home folder, report sheets).
  • Complete and maintain up-to-date paperwork, including care plans, risk assessments, medication forms, and reviews.
  • Manage and monitor care packages to ensure continuity and quality of service.
  • Record and escalate issues through Care Planner and notify relevant stakeholders.
  • Communicate professionally with representatives, professional networks, and family members.
  • Ensure all complaints are handled in line with the company’s complaints procedure.
  • Support the emotional, physical, spiritual, and material wellbeing of Service Users.
  • Cover care calls when necessary.
